Crossroads provides the most convenient, comprehensive, high-quality care to create superior outcomes for those with substance use disorders. Our evidence-based treatments include FDA-Approved Medications, Peer Support, Measurement-Based Care, Care Coordination, Contingency Management and Counseling. Our services include an individualized, whole-person treatment plan built specifically for each patient’s needs.

Clinical Assessment

Patients will see their provider, along with a care coordinator or counselor on their initial and subsequent visits. We build customized, evidence-based treatment plans for each patient. The number of assessments and visits will vary based on the patient’s needs.

Treatment Medication

Crossroads offers a full array of FDA-approved treatment medications, including methadone and buprenorphine, as well as injectables such as SUBLOCADE® and BRIXADI®, which are taken on a weekly or monthly basis. Our providers will work with each patient to customize the best treatment option based on the patient’s individualized care plan.

Counseling

Patient-centered counseling addresses the connection between substance use and co-occurring mental health conditions. We provide behavioral therapy that is used to develop interpersonal and coping skills, support motivation and promote long-term recovery.

Care Coordination | Unmet Social Needs

Our care coordinators work in partnership with a broad variety of community services. Our patients with unmet social needs can visit our Patient Resources page for access to free or reduced-cost food, housing, transportation and more.

Peer Support

Interacting with a certified recovery or peer support specialist is a peer-guided experience led by individuals who have struggled with substance use and are now in recovery. These specialists offer confidential guidance, support, motivation and encouragement while serving as accountability partners to our patients in recovery.

Specialty Populations

We offer support services, such as treating pregnant patients and patients with Hepatitis C. Treatment medications have both been shown to be safe and effective treatments for SUD during pregnancy and also reduce prenatal withdrawal. For our patients with co-occurring Hepatitis C, Crossroads provides regular screenings and treatment.

Financial Services

We take Medicaid, Medicare, TRICARE, VA CCN and most commercial insurances. We will verify benefits and can help patients sign up for insurance through our 24/7/365 Patient Access Center. We offer self-pay options and may also have grant funding available for use.
