What to Expect During Your Initial Visit

On your first visit, you will meet with a dedicated provider and, in many cases, a care coordinator or counselor . This initial consultation is critical in understanding your unique health needs, challenges and goals. The provider will conduct a thorough assessment, which includes:

Medical History: Reviewing your past and current health conditions, substance use history and any relevant medical records.

Behavioral Health Evaluation: Assessing your mental health, emotional well-being and any co-occurring conditions that may impact your treatment.

Social Determinants of Health: Identifying any external factors such as housing, employment or family support that may influence your recovery process.

Ongoing Assessments and Visits

Primary Providers: To monitor physical health and manage any medications or medical interventions.

Care Coordinators: To help connect you with additional resources, including mental health services and housing assistance.

Counselors: For ongoing behavioral health support, Crossroads counselors may use Cognitive Behavioral Therapy (CBT) and Motivational Interviewing (MI) to help you manage challenges and stay on track with your recovery goals.

Customized, Evidence-Based Treatment Plans

Medical Interventions: Medications or therapies to support detox, reduce cravings or treat co-occurring conditions.

Behavioral Therapies: One-on-one sessions to address the psychological aspects of recovery.

Support Services: Harm reduction tools, peer support and access to community resources to help you stay engaged in recovery.
