  • Social Determinants of Health: Identifying any external factors such as housing, employment or family support that may influence your recovery process.

Ongoing Assessments and Visits

Your care doesn’t stop after the first visit. We continually assess your progress and adjust your treatment plan to ensure you are on the best path to recovery. The frequency of visits and assessments will depend on your individual needs, but each encounter is designed to provide personalized care and support. Patients may continue to meet with:
  • Primary Providers: To monitor physical health and manage any medications or medical interventions.
  • Care Coordinators: To help connect you with additional resources, including mental health services and housing assistance.
  • Counselors: For ongoing behavioral health support, Crossroads counselors may use Cognitive Behavioral Therapy (CBT) and Motivational Interviewing (MI) to help you manage challenges and stay on track with your recovery goals.

Creating a treatment plan tailored specifically for you is at the heart of our clinical assessment process. These plans are built on the latest research and best practices in addiction medicine and behavioral health. Your plan will include a mix of:
