BEFORE YOUR APPOINTMENT WHAT TO BRING WITH YOU:
At every appointment you will need your photo identification, insurance card, appointment card, and any applicable co-payment. If any of your information has changed in relation to your address, insurance information, or therapy information, please inform the staff upon arrival so we can update your records. Photo ID + Insurance Card + Appointment Card + Personal Contact Information & Therapy Information + Any Co-Pay = Every Visit
APPOINTMENT DATES/TIMES:
There are legal limits on how many prescriptions a doctor can write each month. When you make an appointment, you are assigned to one of only a certain number of slots available at each clinic session. Therefore, it is very important that you keep your appointments and arrive on time.
TRANSFERRING FROM ANOTHER CLINIC:
If you are transferring from another clinic, please let the Crossroads Harriman staff AND Physician/APP know when your current prescription expires. Failure to do so may result in difficulty obtaining your medication.
PRESCRIPTION MONITORING PROGRAM:
Crossroads Harriman searches all available state Prescription Monitoring Programs (PMPs) as required by law in each state.
